# Wrong facts, hallucinations and careless mistakes (`act.accuracy`)

Feedback Bench, personal agents, built 2026-10-19, window 2026-08-31 to 2026-10-18. Web page: https://feedbackbench.com/#/criterion/act.accuracy

Area: [Getting tasks done](https://feedbackbench.com/criteria/act.md)

**Definition.** Whether the information and outputs the agent gives are correct, or contain made-up facts and errors that need checking. The post names a wrong answer, a mistake or a correct result that was checked.

**Boundary.** Not this: use general.unspecific for 'smart' or 'dumb' with no error or result named. Not this: see [Which AI model runs, how good it is, and choosing it](https://feedbackbench.com/criteria/control.model.md) when the post blames or credits the underlying model. Not this: see [Says it did something it did not do](https://feedbackbench.com/criteria/act.false_reports.md) when the false statement is about its own work or abilities. Not this: see [Web research, monitoring and summarising information](https://feedbackbench.com/criteria/act.research.md) when the point is how thorough or useful the findings are, not that they are wrong. Not this: see [Following instructions, rules and corrections](https://feedbackbench.com/criteria/act.instructions.md) when it ignores what it was told.

Rated user-weeks, all agents: 1545. Complaint share: 69%.

## The brief

Written by large language models from this page's numbers and the posts it quotes, then checked: every number in the text is in the build. Interpretation, not measurement.

Muse and Hark rank highest for getting facts and details right. Their users say answers come out correct and rarely need a second check. This is the 15th most rated of 53 criteria for AI personal agents. Most posts about it are complaints: 68.8% of rated user-weeks complain. OpenClaw, ChatGPT Dots and Gemini Spark rank low. OpenClaw places 7th of 7. Gemini Spark users rate this criterion 1.53 times as often as other users.

### What separates the best from the rest

Users of Muse and Hark describe output they can trust. They say they don't need to fact-check it and that it catches errors in documents. They also say its estimates and listings hold up when they check them. Users of the low-ranked agents describe the reverse: hallucinations, mistakes that turn up when another tool checks the work, and constant error correcting. Some say the agent calls them by the wrong name or misses a receipt in their inbox. Gemini Spark has 3 praise to 37 complaints.

## How it breaks

Rule: A separate pass sorts complaints into failure modes. For each criterion with at least 30 complaining user-weeks, a large language model reads 150 complaint posts drawn at random and names 4 to 8 failure modes; a second large language model then puts every complaint post on that criterion in one mode, or in Other when it fits none. Modes are fixed once named, and new posts are sorted into them each week. A mode counts distinct user-weeks with a complaint in it, per agent; across agents, one user-week per agent. Modes do not change the labels or the Feedback Score. Examples: up to 3 posts per mode from different users, chosen and checked as receipts are.

The largest failure is frequent basic mistakes that force users to check every output. It accounts for 30% of complaining user-weeks, and Grok Bot users account for 31% of it. Wrong search and price results belong mostly to Muse, with 65% of that mode. In off-topic or garbled replies, Hermes Agent has the largest share at 41%.

1066 complaining user-weeks.

| Failure mode | User-weeks | Posts | Agents (user-weeks) | Definition |
|---|---|---|---|---|
| Frequent basic mistakes needing constant checks | 322 | 327 | Grok Bot 101, Muse 77, OpenClaw 49, Hermes Agent 44, ChatGPT Dots 27, Gemini Spark 16, Hark 4, Poke 3, Instinct 1 | Users report a high general error rate on simple tasks, so every output has to be verified or corrected, often with a sense that quality has dropped over time; complaints that are only about cost, speed or setup bugs, with no wrong output, are excluded. |
| Fabricated facts stated with confidence | 155 | 161 | Muse 62, Grok Bot 42, Hermes Agent 15, OpenClaw 14, ChatGPT Dots 9, Gemini Spark 8, Hark 2, Poke 2, Instinct 1 | The agent invents facts, sources, licences, events or content and presents them as true without hedging, such as made-up fixtures, imaginary words or a fictional book; answers that are only shallow or unhelpful but not false are excluded. |
| Off-topic or garbled responses | 73 | 74 | Hermes Agent 30, Grok Bot 16, Muse 10, OpenClaw 8, ChatGPT Dots 5, Gemini Spark 3, Hark 1 | The agent answers a question that was not asked, adds unrelated content mid-task, or outputs nonsense or mixed-language gibberish; responses that are on topic but factually wrong are excluded. |
| Claimed success that never happened | 58 | 64 | Muse 20, Grok Bot 16, OpenClaw 10, Hermes Agent 6, ChatGPT Dots 4, Hark 2 | The agent says a task was done, checked or confirmed when the edits never happened, contain errors (such as a wrong delivery address or a mangled document) or broke something, and the user finds out later; tasks the agent openly reports as failed are excluded. |
| Inaccurate search and price lookups | 54 | 59 | Muse 35, Grok Bot 5, Hermes Agent 4, Hark 3, ChatGPT Dots 2, Gemini Spark 2, OpenClaw 2, Instinct 1 | Web, shopping or listing research returns wrong results, such as prices that are not the cheapest, homes already sold, scam listings, wrong email domains, or claims about a page made before it loaded; failures to reach a site at all, such as captchas or login blocks, are excluded. |
| Wrong numbers from connected user data | 53 | 54 | Muse 22, Grok Bot 17, Hermes Agent 8, OpenClaw 4, ChatGPT Dots 2 | The agent misreads or miscalculates the user's own data, giving wrong step counts, bill splits that change on every try, bad accounting entries, the wrong hardware specs or a wrong address taken from a linked app; errors in general world knowledge are excluded. |
| Stale or wrong date and time | 37 | 38 | Muse 15, Grok Bot 11, OpenClaw 5, Hermes Agent 3, ChatGPT Dots 2, Poke 1 | The agent gives outdated information as current or miscalculates dates, times, weekdays or time zones, such as last year's matches, an event placed on the wrong day or the server's clock used instead of the user's; facts that are wrong for reasons unrelated to time are excluded. |
| Other | 321 | 338 | OpenClaw 72, Grok Bot 70, Muse 68, Hermes Agent 62, ChatGPT Dots 36, Gemini Spark 8, Hark 4, Instinct 1 | Complaints that fit none of the modes above. |

## Top requests

What users ask to add or change, most asked first. 190 user-weeks ask for something. Requests do not change the Feedback Score. Rule: A separate pass by a large language model reads every counted post and extracts what the user asks the agent or its vendor to add or change, with the criteria it maps to and a short normalised wording; it does not touch the labels or the Feedback Score. A large language model groups the wordings within each criterion (the first criterion the request maps to) into themes, and a second pass merges themes that ask for the same change under different criteria; a merged theme sits under the criterion most of its requests map to, and code counts it once. A theme counts distinct user-weeks that ask for it, per agent; across agents, one user-week per agent. Themes asked in fewer than 2 user-weeks, and requests that share no theme, are not shown. Examples: up to 3 posts per theme from different users, without slurs: whole posts of 60 to 450 characters with at least 8 words beyond handles and links, posts of at most 280 characters first, then the newest.

## Posts

Receipts rule: Up to 5 praise and 5 complaint posts per area and 3 per criterion. Each is a whole post of 60 to 450 characters (700 for areas) with at least 8 words beyond handles and links, that judges the criterion, whose labels are all praise or all complaint, and that contains no slur. Posts that touch the fewest criteria come first, then posts of at most 280 characters, then the newest. A second large language model reads the first 3 candidates per slot and keeps a post only if it clearly praises, or clearly complains about, the agent on that criterion. Links and personal details removed by the source show as marks such as [link].
